Seabury Active Living Retirement Community, and Seabury at Home, Bloomfield

One of Connecticut’s first retirement communities, Seabury has grown impressively from its original facility. The sprawling campus now boasts several individual residences, from apartments to substantial single homes. Their memory care unit is superb. The surroundings provide lovely views in all seasons. An immense fitness facility, including a large pool promotes continued fitness of older adults. Seabury boasts an impressive participation of its residents in the fitness facility. Seabury also has an innovative Life Care program and Seabury at Home service. Their home care program provides both Medicare as well as home private duty services for the community and surrounding area. Their health care center boasts state of the art short-term rehab in a beautiful setting. There is excellent coordination with their Home Care agency, which provides nursing, and all therapies, following a comprehensive home safety assessment prior to discharge. It is a privilege to work in co-operation with them.

Seabury also has an innovative Life Care program and Seabury at Home service. This program allows seniors to remain in their homes while taking full advantage of the full scope of services at Seabury.

McLean Hospice and Palliative Care, Simsbury

McLean Hospice is dedicated to improving patients’ comfort and supporting their families. Their team in helps patients you live as fully and comfortably as possible so your last days may be spent with dignity and quality. Our goal is to provide satisfying days, fear-free nights and the richness of time with your loved ones, surrounded by a supportive team. Hospice nurses are available to you 24 hours a day, 365 days a year.

Their hospice team includes highly experienced, board-certified nurses, medical social workers, spiritual counselors and dedicated volunteers all committed to improving the end-of-life journey through comfort-focused patient care and support for grieving families. Bereavement services and support are available for up to 13 months.

Founded in 1997, Seasons is the fifth largest Hospice provider in the nation. Providing high quality clinical and compassionate care to patients and families in 15 states through 20 Medicare-certified home care clinical operations and 10 Hospice Inpatient Centers. In 2012, Seasons caregivers served more than 16,000 patients of all ages totaling more than 1 million patient days.

Atria, Hamilton Heights, West Hartford

This charming and impressive community is located in a beautifully restored, historic building situated in one of New England’s finest neighborhoods. Built for a wealthy family in the late 19th century, and later the Mount Saint Joseph Academy – affectionately known as “The Mount” – this marble-columned masterpiece is now home to fascinating older adults with a few tales of their own to tell.

We are impressed by a few outstanding differentiators: the chef quality meals and their adaptability in altering meals at residents’ requests. The enormous array of activities. The friendliness of staff.. The nursing staff is helpful and responsive 24 hours a day, even as we discovered, on a holiday. And, perhaps our favorite …A pet friendly environment!

Beacon Hospice, East Hartford

In a state with many fine hospice agencies from which to choose, Beacon distinguishes itself in its special program for people with dementia. The needs of patients and family members caring for people with dementia at the end of life is especially challenging. And the Beacon staff is indeed experts on this special group of patients. Beacon goes above and beyond by supporting not only the patient but the family and caregivers as well through this journey.

Their website offers a short documentary based on Atul Gawande’s, Being Mortal, the award-winning book about navigating through important but difficult choices near the end of life. It is a superb, enlightening introduction to hospice. The resources on their website site also include a brief questionnaire to determine if a loved one may be ready for a hospice evaluation.
